SOLE PROPRIETOR/SINGLE MEMBER LLC Waiver - Workers Compensation Coverage and Benefits
I am a Sole Proprietor/Single Member LLC
I am performing work as an independent contractor for Floor Daddy, LLC.
I am not an employee of Floor Daddy, LLC for workers compensation purposes.
Therefore, I am not entitled to workers compensation benefits from Floor Daddy, LLC.
I understand that if I have any employees working for me, I must maintain workers compensation insurance on them.
